Bipolar disorder involves more than ordinary mood changes. Recognizing Possible Mood Episodes

Depressive episodes may involve low mood, loss of interest, fatigue, hopelessness, sleep or appetite changes, and difficulty functioning.

Manic or hypomanic symptoms may include:

  • needing much less sleep without feeling tired
  • feeling unusually energized, powerful, confident, or irritable
  • speaking more rapidly or having racing thoughts
  • starting many activities or taking on unrealistic commitments
  • becoming more impulsive with money, sex, substances, driving, or major decisions
  • feeling unusually distractible or agitated
  • behavior that others recognize as significantly different from your baseline

Hypomania can initially feel productive or positive, which sometimes makes it difficult to recognize. A careful history is especially important because bipolar depression can resemble major depression, while treatment considerations may differ.

Ongoing Psychiatric Care Matters

Evaluation includes discussion of depressive and elevated mood periods, sleep, energy, impulsivity, psychosis symptoms, family history, previous treatment, substance use, hospital care, current medications, and safety. Treatment may involve medication management, brief support, mood and sleep tracking, lifestyle planning, coordination with a therapist, and referral for additional care when needed.

Medication monitoring may include discussion of effectiveness, side effects, adherence, interactions, and any laboratory or medical follow-up required for a particular treatment. Never start, stop, or change bipolar medication without guidance from a qualified prescriber.

    What is the difference between bipolar I and bipolar II disorder?

    Bipolar I disorder includes at least one manic episode. Bipolar II disorder includes hypomanic episodes and major depressive episodes, without a full manic episode. Both can significantly affect functioning and require accurate assessment.

    Why does a provider ask about sleep when evaluating bipolar disorder?

    A reduced need for sleep can be an important sign of mania or hypomania. Sleep disruption can also precede or worsen mood episodes, making it valuable to monitor.

    Can bipolar disorder be managed through telehealth?
